Simplifying LASIK Basics – Part 2: What’s Involved

Welcome to the second in our Simplifying LASIK Basics series. In this edition, we’ll highlight what the procedure’s like and types of LASIK in a way that should keep it simple. Hopefully it can help you determine what types of questions to ask your doctor as part of your decision to have LASIK performed.

What’s The Procedure Like? Although there’s more to it than this, here are the basics. Big Picture: Laser technology reshapes your cornea so things look sharper and clearer. Who It’s For: Mainly for anyone who is nearsighted, farsighted or has astigmatism. What Happens: A “flap” is created in the cornea, then the flap is gently lifted and folded back, and the cornea is reshaped using laser pulses to correct focusing problems in the eye. On average, it takes approximately 5 minutes, and may take slightly longer if your vision problems are more severe.

Types Of LASIK. Essentially, there are three basic types of LASIK, each designed to do what LASIK is supposed to do – help you see better, possibly better than you ever have with less dependency on glasses or contact lenses. You may have seen types of LASIK referred to this way: Conventional LASIK, Wavefront-Optimized LASIK, and Wavefront-Guided LASIK (also called Custom LASIK).

Which Type Of LASIK Is Best? Different doctors may use different types for different reasons. At Eye Consultants of Texas, we use a system based on Custom LASIK. It brings together two highly advanced technologies for a procedure branded “iLASIK.” Dr. Labor selected this after careful evaluation, mainly because it has proven to be more efficient and successful than other LASIK technologies he has used. But also, because of what it was designed to do – give the surgeon more control in customizing the procedure to each patient’s eye to get the best results. While the technology itself is extremely important, it’s even more important that your doctor has the skill and expertise to use it! Having been the first in the U.S., Texas, or locally to perform new procedures, Dr. Labor does his homework, and trains extensively on new technologies and advancements before ever putting them to use.

Supreme Court Health Care Law Hearings

In November of 2011, the Supreme Court announced that it would hear challenges to President Obama’s health care reform law (Patient Protection and Affordable Care Act of 2010).  Those hearings are scheduled for March 26-28 and will address the following issues:

  • Are opponents of the new health care law challenging the law too soon?  According to the new law, everyone is required to purchase health insurance, with or without government assistance, or be fined (individual mandate).   However, it is unclear as to whether the fine to be imposed is considered a tax.  If it is a tax, then the Anti-Injunction Act, a federal law, prohibits anyone challenging the health care law until 2014, when violators will be required to pay the “fine” or “tax”.
  • Assuming the individual mandate can be challenged now, is the individual mandate constitutional?   There is a Commerce Clause in the Constitution, which allows Congress to regulate interstate commerce, but does this clause also allow Congress to require people to purchase health care insurance?
  • If the individual mandate is found to be unconstitutional, will the rest of the health care law remain in effect?   Sometimes a portion of the law can be struck down, if it is found to be unconstitutional, while the rest of the law continues in force.  If the Court finds that the unconstitutional part of the law is so intertwined with the rest of the law, then it comes to an all or nothing at all proposition.
  • Is the health care law’s expansion of the Medicaid program constitutional?   The argument is state versus federal rights.  The new law requires states to change their Medicaid programs, so that all people will be eligible for coverage.  If states do not change their laws, they will lose all of their federal funding for the program. 

The Supreme Court plans to release recordings of the arguments on its website (http://www.supremecourt.gov/) around 2 p.m. each day for arguments held that morning and around 4 p.m. for arguments held in the afternoon of March 28.   After hearing the challenges to the new health care law, the Court is expected to reach its decision in June 2012.

Premium smart lens patients the real stars of our latest video

Eye Consultants of Texas has just launched a new video featuring actual Fort Worth metro area patients telling their stories. Simplifying LASIK Basics – Part 3: Safety and Results

Is LASIK just a passing thought? Or, are you ready to jump in with both feet, so to speak? Either way, two of the first things to consider are safety and what types of results you can expect. Continued advancements make LASIK safer and more effective than ever before. Still, LASIK is surgery, and you owe it to yourself to choose a reputable doctor and practice, who, in turn, owe you a full disclosure – standard procedure at Eye Consultants of Texas. As the third in our Simplifying LASIK Basics series, here are a few highlights about what you should know.

How Safe Is LASIK? Any surgery comes with risks, and amazing advancements have proven LASIK to be one of the safest surgical procedures today. At Eye Consultants of Texas, we won’t perform LASIK if we determine a patient is at high risk, which, according to the American Academy of Ophthalmology (AAO), is low. Generally speaking, AAO suggests that 2% or less of all patients have complications or side effects; from discomfort or difficult vision (that usually disappears over time), to more extreme cases where it can last longer. A thorough, necessary examination can reveal potential risks, and determine if you’re a candidate for LASIK at all. If it turns out that you are not a candidate, there are other vision correction options you and your doctor can discuss.

What Types Of Results Can You Expect? If you’re a LASIK candidate, it must be medically verified that you can achieve a level of visual improvement that makes the surgery worth it in the first place. Everyone expects to see 20/20 without corrective eye wear, and a majority of patients achieve this. There are also cases where the result may be less than 20/20. This can depend on how severe your eyesight is prior to surgery, which you can discuss with your doctor. Our LASIK patients generally tell us that they can see better without glasses or contacts than with them before they had the procedure. Once again, your doctor should only perform LASIK if he or she honestly feels you really need it.

How Quickly Does Your Vision Improve? Most of our LASIK patients are amazed at how well they can see right after surgery. Vision should continually (and gradually) improve over the next few days, and continue to improve after that. The thing to remember: your eyes will need time to adjust and heal. There may be some mild “scratchiness” and discomfort right after surgery. This should go away, and prescribed medication will help with discomfort and healing. Necessary follow-up visits carefully monitor your progress, to ensure proper healing and that there are no complications.

Can You Ditch Glasses And Contacts Altogether? LASIK isn’t permanent, and, in some instances, glasses may be needed for reading or seeing up close. The natural aging of the eyes can also have an effect. So why have it in the first place? Most people who have LASIK can see a noticeable improvement in their vision for many years, with far less dependency on corrective eye wear for simple day-to-day tasks. Which can be life-changing if you’ve had to rely on glasses or contacts for most (or all) of your life! Baltimore Eye Surgeon Now Performing Cosmetic Eyelid Surgery

Baltimore, MD – According to the Administration on Aging (AOA), persons 65 and older represented nearly 13% of the U.S. population in 2009, and are expected to represent 19% by 2030. The Bureau of Labor Statistics finds that the number of older Americans in the workforce is growing as well. As the population and workforce ages, Baltimore eye surgeon Jay C. Grochmal, MD says his patients that are late stage baby-boomers are under extreme pressure to maintain a high level of performance in the workplace, and portraying a vivacious and youthful appearance is essential to continued success.

Breast Cancer Awareness Month

October is National Breast Cancer Awareness Month. According to the CDC, 210,203 women in the United States were diagnosed with breast cancer, and 40,589 women died from the disease, in 2008 (the latest year for which statistics are available). With the exception of skin cancer, breast cancer is the most common cancer among American women.

Breast cancer screening can often lead to earlier detection and treatment of the disease. Most health insurance companies pay for the cost of breast screening tests.  For those people worried about the cost, the National Breast and Cervical Cancer Early Detection Program (NBCCEDP) offers free or low-cost mammograms and education about breast cancer.  You can find out if you qualify here.

In recognition of Breast Cancer Awareness Month, we are sharing a story of a breast cancer survivor.  Kathleen Tebb, a 39-year-old mother of two children, was diagnosed and had a mastectomy followed by aggressive treatment, including radiation.  With the help of physical therapy, Kathleen was eventually able to resume one of her passions—rock climbing.  Read more about Kathleen’s story here, and check out an interview with her below.

Mayo Clinic study confirms stents are safe for patients allergic to metal

Mayo Clinic researchers have found that cardiac patients who have skin allergies to metals commonly found in jewelry, such as nickel, are at no higher risk for complications if they receive a stent containing these metal components.

Mayo Clinic cardiologist Rajiv Gulati, M.D., Ph.D., and colleagues studied a total of 29 patients with a history of skin allergies to stent metal components who subsequently underwent coronary stent implantation. The research team compared clinical outcomes with a matched control group of 250 non- metal allergic patients who received similar stents. In addition to following the study patients’ outcomes in the long term, the team reviewed blood to look for signs of allergic reactions.

Medicare And Medicaid Overpayment Suit Upheld

Whistleblowers, Lucas Matheny (“Matheny”) and Deborah Loveland (“Loveland”), brought an action against Medco Health Solutions (“Medco”) claiming that Medco and its subsidiaries failed to report and refund to the federal government $69 million in Medicare and Medicaid overpayments.  The plaintiffs, former employees of Medco, used the authority of the False Claims Act to bring their lawsuit.  The False Claims Act allows private individuals to sue an entity on their own and the government’s behalf.  In doing so, the individuals receive compensation for being a “whistleblower”, which is usually between 15% and 25% of the funds recovered, including reasonable expenses, attorney fees and costs.

Matheny and Loveland knew of the overpayments to Medco, but were told that because of insufficient manpower to process the necessary forms, the overpayments would not be refunded to the government.  Matheny and Loveland claimed the overpayments were transferred to fictitious patient accounts and perfect records with a zero percent error rate were created using a specific computer program.   In addition, Medco filed a false certificate of compliance with the government, knowingly concealing millions of dollars in overpayments. 

When the lawsuit was first heard by the court, it was dismissed, due to the court noting that the plaintiffs’ complaint had been amended three times and failed to state a claim. However, on appeal, the court concluded that Matheny and Loveland had sufficiently alleged the existence of a false record.  Additionally, the court held that the defendants (Medco) knew the records were false and were to be used to avoid a financial obligation to the government.

The Matheny v. Medco Health Solutions matter is a prime example for healthcare providers to be careful with potential overpayments and how to handle those overpayments should they occur.

Cholera in Cuba

The first outbreak of cholera in over a century has been reported by the Cuban Ministry of Public Health. The risk to travellers is low, but they should be aware that Cholera can be spread via food being prepared by a person who is ill with the disease.

Travellers are advised to take care with food and water, and to ensure they eat seafood such as crabs and crayfish only when it has been cooked through and is thoroughly hot.

Rubella (German measles) in Japan

The total number of cases of rubella reported in Japan in the month of July alone was equal to the total number of cases in 2011. Cases have occurred in Tokyo, Osaka, Kanagawa and Kyoto.

Travellers are advised to consider having MMR vaccine if they have not had the disease or been immunised against it. Two doses of the vaccine give good protection.

Advancing Care for Advanced Illness

Millions of Americans are dealing with advanced illness—defined as when health conditions become serious enough that general health declines, and treatment loses its impact.  A significant challenge is that the quality of care these individuals receive varies significantly.

The Coalition to Transform Advanced Care, otherwise known as C-TAC, is an organization focused on ensuring everyone, especially the sickest and most vulnerable, receive high-quality care that is consistent with their values and respects their dignity.  They hope to transform care for advanced illness through public and private policy, making changes to the care delivery system, and by empowering consumers. C-TAC is bringing together patient advocacy groups, medical teams, faith communities and individuals in an effort to bring about this change as quickly as possible.

To shed more light on these issues and explore potential solutions, C-TAC is hosting a summit on advanced illness care in Washington, D.C., January 29-30, 2013.  The summit will explore community-based approaches, cultural issues, public perspectives, and the economics of advanced care.  You can read more about the event here.

Is LASIK Right for Me?

As an ophthalmologist in Winchester, one of the top questions I’ll hear time and time again is, “Is LASIK right for me?”  And, simply put, there’s no simple answer!  Only a qualified medical professional can determine if LASIK’s right for you.  There are, however, certain factors that good LASIK candidates share.

Not sure where to look?  To get you started, here are a few questions to ask yourself:

  • Are you in good health? Good candidates for LASIK are healthy and free of autoimmune disorders such as Lupus, HIV and muscular sclerosis.
  • Do you have any eye conditions? Certain eye conditions are not compatible with laser eye surgery.  Often, these include cataracts, chronic corneal infections or keratoconus.
  • Are you the right age? In the US, LASIK is FDA approved for patients over the age of 18.  On the other hand, if you’re worried you’re too old for a LASIK procedure, consider this:  Reporting on a recent study on LASIK in older patients, WebMD explains that, “Patients in their 60s were just as likely to have good [LASIK] outcomes as those in their 40s and 50s.”
  • Is your dependence on glasses or contacts holding you back? For many patients, reducing their dependence on glasses and contact lenses is a major reason they’re exploring LASIK in the first place.  A lot of folks find glasses and contacts to be uncomfortable, inconvenient and unattractive.  And, while it can’t guarantee that you’ll never need to put on a pair of glasses again, LASIK will very often reduce a patient’s dependence on them.
  • Are you ready to learn more? Finally, a good LASIK candidate is an informed LASIK candidate.  Be sure to understand the potential risks and benefits of the procedure, maintain realistic expectations, and most importantly, schedule a consultation with a qualified LASIK surgeon.

Top 5 Myths of Glaucoma

Most people have heard of glaucoma, but few know the whole story.  In short, glaucoma is an eye condition where the optic nerve becomes damaged, usually as a result of increased pressure in the eye.  This damaging of the optic nerve ultimately causes a gradual but serious loss of vision.  Untreated, the disease often results in complete blindness.

Are You Turned On?

Are you turned on?…electrically speaking, that is…and I’m not refering to Timothy Leary’s, “Turn On, Tune In, Drop Out.” 

The other day we talked about how we are electrical beings and that from time to time we need to “ground” ourselves and release some of the static electricity that is built up in our bodies.  I had a couple of people tell me that this is done all the time throughout most of Europe.  I also had a few people who wanted to see learn more about how our bodies operate on an electrical current.

If you’re one of our readers who would like a visual of the body producing electricity in action then this video is for you.
 

What you are viewing is a group of  four people holding hands and two of the people are holding “hands” with a Ring Around The Rosie Doll.  On each of the dolls hands is a small metal button.  When both of these buttons are touched with bare skin the doll begins to sing.  You will notice in the video that there are a couple of times that the participants let go of the hand of one of their partners…once they do this the doll stops singing.  Once they hold hands again the doll starts to sing.  The doll sings because of the electricity being produced by each participant. 

Remember, you are an electrical being…do something good for yourself to recharge your batteries…a nice walk around the block in the evening air is always a simple (and free) thing that you can do to bring in more good energy and release the bad.

Simplifying LASIK Basics – Part 1: Starting Points

As we’ve mentioned here before, LASIK has become a household word to anyone relying on glasses or contact lenses for everyday things. There’s no shortage of info out there, and much of it can be confusing, or written like an owner’s manual. At Eye Consultants of Texas, we cover the bases in educating our Dallas/Fort Worth LASIK patients, and try to do this as simply as possible. With that in mind, this is the first in a series of blogs we’ll be posting to highlight the basics of LASIK, in hopes of putting LASIK medical-speak more into simple-speak.

First Things First. LASIK is a simple, painless procedure that can be performed quicker and safer than ever before, mostly thanks to technological advancements. Sometimes they opt for LASIK so they can see better and look better, or simply because they’re tired of having to wear glasses all the time. Praise From a Visiting Former Navy Seal and Best-Selling Author

We were recently privileged to have New York Times bestselling author, and former Navy SEAL, Brandon Webb, speak at Eye Consultants of Texas in Grapevine. It was the idea of founder Phillips Kirk Labor, MD, to host this event, primarily for his staff and a group of local law enforcement officers, as well as special guests.

A highly sought-after motivational speaker, Brandon focused his talk on the importance of attention to detail and striving for excellence. His New York Times bestselling book, The Red Circle, shares his personal story of dealing with adversity in a positive and constructive way, and passes on valuable leadership lessons he learned as a Navy SEAL. Everyone in attendance at our practice was incredibly inspired, just like all of Brandon’s audiences in his speaking engagements across the U.S.

 

In addition to inviting him to speak, Dr. Labor invited Brandon into the operating room to observe actual cataract procedures. Recently, Brandon wrote about his Eye Consultants of Texas visit and experiences in his blog.

 

Here is an excerpt from Brandon’s blog:

This unpleasant viral illness is spread by infected mosquitos that bite during daylight hours, and there is no vaccine available at present.

Is LASIK Right for Me?

As an ophthalmologist in Winchester, one of the top questions I’ll hear time and time again is, “Is LASIK right for me?”  And, simply put, there’s no simple answer!  Only a qualified medical professional can determine if LASIK’s right for you.  There are, however, certain factors that good LASIK candidates share.

Not sure where to look?  To get you started, here are a few questions to ask yourself:

  • Are you in good health? Good candidates for LASIK are healthy and free of autoimmune disorders such as Lupus, HIV and muscular sclerosis.
  • Do you have any eye conditions? Certain eye conditions are not compatible with laser eye surgery.  Often, these include cataracts, chronic corneal infections or keratoconus.
  • Are you the right age? In the US, LASIK is FDA approved for patients over the age of 18.  On the other hand, if you’re worried you’re too old for a LASIK procedure, consider this:  Reporting on a recent study on LASIK in older patients, WebMD explains that, “Patients in their 60s were just as likely to have good [LASIK] outcomes as those in their 40s and 50s.”
  • Is your dependence on glasses or contacts holding you back? For many patients, reducing their dependence on glasses and contact lenses is a major reason they’re exploring LASIK in the first place.  A lot of folks find glasses and contacts to be uncomfortable, inconvenient and unattractive.  And, while it can’t guarantee that you’ll never need to put on a pair of glasses again, LASIK will very often reduce a patient’s dependence on them.
  • Are you ready to learn more? Finally, a good LASIK candidate is an informed LASIK candidate.  Be sure to understand the potential risks and benefits of the procedure, maintain realistic expectations, and most importantly, schedule a consultation with a qualified LASIK surgeon.

Mayo Clinic study finds that cardiac rehab reduces mortality by 45 percent

Randal J. Thomas, M.D., director of Mayo Clinic’s Cardiovascular Health Clinic, and colleagues found that patients who participate in cardiac rehabilitation after having heart interventions such as angioplasty, stents and clot-busting drugs have a 45 percent lower mortality rate.

The research team studied data from more than 2,300 patients between 1994 and 2008, and recently published the results in Circulation 

Only about 40 percent of patients in the study participated in cardiac rehabilitation. These findings are particularly important for interventional cardiologists, Dr. Thomas says, because encouraging patients to pursue cardiac rehab after their procedure can potentially save more lives than previously thought.
