Careers As Advanced Health Care Practitioner In Demand

According to data collected by the American Medical Group Association’s and Cejka Search 2011 Physician Retention Survey, careers in the advanced health care field are growing rapidly.  As such, employment in medical groups for nurse practitioners (“NPs”) and physician assistants (“PAs”) has increased significantly in the past five years.  According to the data, 75% of the responses to the survey indicated more NPs and PAs will be hired in the next five years.

Growth in these advanced health care professions is due largely to economics, according to an article in U.S. News and World Report.  NPs, for example, can provide patients with many primary health care services at a fraction of the cost of an internist.  In addition, FiercePracticeManagement reports that due to physician shortages and the demand to create a medical “team” to care for patients (accountable care organizations, “ACO”), medical groups are employing the NP and PA at increasingly high rates.  This gives NPs and PAs the advantage of choosing where to work, thus creating a high turnover rate for these positions.  In addition, recruiting and retaining these professionals are challenges to medical groups.

Experts Weigh in on Preventing and Addressing Obesity

As National Childhood Obesity Awareness Month comes to a close, it’s clear that childhood obesity is an issue supported by communities, parents, schools and community programs around the nation.

Recently, the East Bay Community Foundation partnered with Kaiser Permanente to present the Weight of the Nation—a national public health campaign targeting obesity—at the Council on Foundations’ annual community foundations conference in New Orleans. The East Bay Community Foundation is an important partner in the work to combat obesity in our local communities, and the organization demonstrates how community foundations nationwide can lead collaborative efforts in fighting obesity. In this interview, Nicole Taylor, president and CEO of the East Bay Community Foundation, explains the challenges in Northern California that the East Bay Community Foundation and Kaiser Permanente are working to address.

Although Colorado takes pride in being the leanest state in the nation, the rate of overweight and obesity among children in Colorado is on the rise. In addition, the rate of childhood poverty is rising faster in Colorado than everywhere else in the nation. Children living in poverty are faced with a number of barriers putting them at greater risk for becoming overweight or obese including less access to nutritious foods like whole grains, fruits, and vegetables.

Over the last few years, The Denver Foundation and Kaiser Permanente have both supported a nonprofit aimed at addressing hunger in Colorado. Hunger Free Colorado, formerly known as the Colorado Coalition to End Hunger, is the state’s leading advocate to end hunger. It aims to do this by increasing participation in federal nutrition programs, food stamps, and school breakfast and lunch programs.

In this interview, Barbara Berv, vice president of philanthropic services at The Denver Foundation, speaks about their partnership with Kaiser Permanente and Hunger Free Colorado and the goal of eliminating hunger in Colorado.

The City of New Orleans has been taking bold steps to address obesity and fitness on a local level—most especially for the city’s children. In this recent interview, Dr. Karen DeSalvo, City of New Orleans health commissioner, discusses the Fit NOLA Partnership, which aims to improve fitness levels and combat childhood obesity in New Orleans through a number of innovative, data-driven program and policies. DeSalvo’s vision is that through Fit Nola, New Orleans will become one of the top ten fittest cities in the United States by 2018.

Supreme Court Health Care Law Hearings

In November of 2011, the Supreme Court announced that it would hear challenges to President Obama’s health care reform law (Patient Protection and Affordable Care Act of 2010).  Those hearings are scheduled for March 26-28 and will address the following issues:

  • Are opponents of the new health care law challenging the law too soon?  According to the new law, everyone is required to purchase health insurance, with or without government assistance, or be fined (individual mandate).   However, it is unclear as to whether the fine to be imposed is considered a tax.  If it is a tax, then the Anti-Injunction Act, a federal law, prohibits anyone challenging the health care law until 2014, when violators will be required to pay the “fine” or “tax”.
  • Assuming the individual mandate can be challenged now, is the individual mandate constitutional?   There is a Commerce Clause in the Constitution, which allows Congress to regulate interstate commerce, but does this clause also allow Congress to require people to purchase health care insurance?
  • If the individual mandate is found to be unconstitutional, will the rest of the health care law remain in effect?   Sometimes a portion of the law can be struck down, if it is found to be unconstitutional, while the rest of the law continues in force.  If the Court finds that the unconstitutional part of the law is so intertwined with the rest of the law, then it comes to an all or nothing at all proposition.
  • Is the health care law’s expansion of the Medicaid program constitutional?   The argument is state versus federal rights.  The new law requires states to change their Medicaid programs, so that all people will be eligible for coverage.  If states do not change their laws, they will lose all of their federal funding for the program. 

The Supreme Court plans to release recordings of the arguments on its website (http://www.supremecourt.gov/) around 2 p.m. each day for arguments held that morning and around 4 p.m. for arguments held in the afternoon of March 28.   After hearing the challenges to the new health care law, the Court is expected to reach its decision in June 2012.

Mayo Clinic offers newly approved treatment for GERD

Daniel Smith, M.D. , chair of the Department of Surgery at Mayo Clinic in Florida, discusses minimally invasive surgical options for patients with gastroesophageal reflux disease (GERD).  A novel device, a ring of tiny magnetic titanium beads that acts to keep stomach acid from leaking into the esophagus, is now offered at Mayo Clinic in Florida. Mayo Clinic in Florida is one of the first health care institutions in the U.S. to offer a newly approved device to treat GERD. Mayo Clinic in Florida helped test the device in patients. The U.S. Food and Drug Administration (FDA) approved the device and treatment procedure on March 22, 2012, for patients with GERD who continue to have chronic reflux symptoms despite taking medication. Mayo Clinic in Florida was one of 14 centers nationally that participated in a clinical trial that led to the FDA’s approval of the device.

About GERD
In patients with gastroesophageal reflux disease, liquid or food in the stomach flow back up into the esophagus due to the inability of a ring of muscle between the lower esophagus and the top of the stomach to close properly. If drugs aimed at neutralizing the acid in the stomach fails to prevent GERD, an operation designed to correct the mechanical defect is considered. Nearly 2 million patients of those patients could benefit from this treatment, which is much less complex than current surgical options, says Dr. Smith.

The results of the clinical study that led to approval of the device have not yet been published. “The data presented to the FDA, however, revealed striking results when compared to other GERD treatments that have been investigated over the past 20 years,” says Dr. Smith. “The system offers effective control of GERD with limited side effects and thus far an excellent safety record.”

The implanted device is a ring of tiny magnetic titanium beads that is wrapped around the junction between the stomach and esophagus, serving as a mechanical augmentation of the lower esophageal sphincter (the ring of muscle). The magnetic attraction between the beads is strong enough to keep the sphincter closed to refluxing acid, but weak enough so that food can pass through it into the stomach. The device can be implanted using minimally invasive surgery methods.

“I expect this device to be a game changer for the treatment of GERD in select patients who have failed management with drugs,” says Dr. Smith.

Kenneth R. DeVault, M.D. , chair of the Department of Internal Medicine at Mayo Clinic in Florida, also participated in the studies. “I have many patients who are searching for something more than medication for their reflux, but have been hesitant to undergo a traditional reflux surgery,” he says. “I think this procedure may well be a very attractive option for that group.”

Drs. Smith and DeVault were consultants to the company that developed the device and participated in the research study. Mayo Clinic licensed related technology to the company in exchange for equity.

CMS To Discontinue Second Overpayment Notice

As any physician knows, handling Medicare and Medicaid payments can be a very complex and time consuming process.  As a result, many physicians, through no fault of their own, occasionally overcharge Medicare and Medicaid.  To recoup such overpayments, the Centers for Medicare & Medicaid Services (CMS) administers the Medicare Overpayment Notification Process.  Until recently, where an alleged overpayment has not been promptly resolved and refunded, CMS would send physicians a series of three letters: an Initial Demand Letter, a Follow-up-Letter 30 days later, and then an Intent to Refer Letter.  However, CMS recently determined that the second letter was unnecessary because a substantial number of physicians responded promptly to the Initial Demand Letter with a payment.  In addition, a recoupment action occurs on average 41 days after the initial letter, and that action is accompanied with another notice regarding the overpayment

Thus, it is important for physicians to respond to the Initial Demand Letter in order to avoid further complications such as a referral to a debt collector.  In fact, if an overpayment is not resolved within 90 days of the Initial Demand Letter, CMS will provide a notice of intention to refer the debt for collection.

Medicare Star Quality Ratings Released

The Centers for Medicare & Medicaid Services recently announced that of 563 Medicare Advantage plans rated nationwide, only 11 Medicare health plans with prescription drug benefit earned 5 stars for 2013 — the highest overall rating for quality and service.

Each year through the Medicare Star Quality Ratings system, CMS rates Medicare health plans (both parts C and D) on a scale of 1 to 5 stars, with 5 stars representing the highest quality. The overall scores are based on more than 50 care and service quality measures across five categories, including staying healthy, managing chronic conditions, member satisfaction, customer service and pharmacy services.

In addition to receiving high-quality care and service, 5-star excellence means that Medicare beneficiaries may enroll in a 5-star plan from Dec. 8, 2012, to Nov. 30, 2013, without having to wait until the next annual enrollment period.

Leading the nation with six of the 11 Medicare health plans earning 5 stars was Kaiser Permanente. The Kaiser Permanente 5-star Medicare plans are those operating in California, the Northwest, Hawaii, Colorado, Ohio and the Mid-Atlantic States. Kaiser Permanente’s Georgia region is the top-rated plan in its area with 4.5 stars.

Rabies Vaccine Shortage in the UK

There is at present an acute shortage of rabies vaccine throughout the UK. There will be no further supplies for some time.

Globe Travel Health Centre has sufficient stocks for the time being. However, to conserve stocks:

  • Rabies vaccine will be available on Tuesday, Thursday and Saturday clinics only.
  • The vaccine will be administered by the intradermal route, using a small dose of vaccine.

This is just as safe and efficiacious as the more traditional intramuscular route, but uses less vaccine.

The World Health Organisation has stated:

Intradermal immunisation is an acceptable alternative to standard intramuscular vaccination. Intradermal vaccination has been shown to
be as safe and immunogenic as intramuscular vaccination, yet requires less vaccine. This alternative should be considered in settings constrained by cost and/or supply issues
’.

Intradermal rabies vaccination costs £30.00 per dose – a course of three doses is required for primary vaccination.

What to expect when considering LASIK

If you’ve ever considered LASIK here in Dallas / Fort Worth, the first thought might have been, “what can I expect?” Like a lot of medical procedures, LASIK surgery has advanced and improved over the years. Today, LASIK is allowing more patients to become less dependent on glasses or contact lenses. It’s not uncommon for our patients to tell us, “my eyesight is better without glasses than it was with glasses before I had surgery.”

Here is a shortlist of the most important things to expect:

1. A thorough examination to see if you’re a LASIK candidate. First, you must be 18 or older to have it performed. People well into their 40’s can be LASIK candidates as well. Age, vision health, and overall health can make a difference.

2. For your doctor to tell you everything you need to know. LASIK has become safer, more effective, and with faster recovery times. Still, LASIK is a surgical procedure, which is why your doctor should shoot straight with you (as ours do) about the benefits and any potential complications. This includes taking the time to answer your questions and listen to your concerns.

3. Skill, experience, and advanced technology. Phillips Kirk Labor, MD is known locally here in metro Dallas, and internationally, as a LASIK innovator, but also for his devotion to each patient’s well being. He uses an advanced procedure called iLASIK, which combines the Intralase IFS Femtosecond Laser with VISX technology. iLASIK was designed to improve results, but also safety, and patient comfort (an added comfort is that we perform the entire procedure in our office).

4. Mild, temporary post-surgery discomfort. The procedure itself involves very minimal or no discomfort, and takes only a few minutes. After surgery, a mild scratchiness, burning or itching is normal, which quickly improves as the eyes adjust and heal. We tell our patients to immediately get rest and use special eye drops we prescribe to speed the healing process.

5. Better vision. Our patients generally see better right after surgery, which continually improves as the days progress. LASIK isn’t permanent, and results can vary. But most every LASIK patient can enjoy clear vision for many years. This is why every LASIK procedure starts with a thorough examination – to ensure the best outcome, but also to inform you of everything that will happen.

Leptospirosis (Weil’s Disease) in the Dominican Republic

In late 2011 a Dutch tourist contracted leptospirosis after he returned home from a holiday in the Dominican Republic. He had choked on muddy water after entering the Chavon river, whist on an excursion. He required admission to intensive care, and spent 10 days in hospital.

Human infection results from exposure to infected urine from mammals, often rats. Tourist should be aware that activities in local rivers may give rise to infection.

Think You’re Too Old for Laser Vision Correction? Study Suggests that LASIK Works in Older Patients

If you’re in your 40s, 50s or 60s, you may think you’re too old to undergo LASIK surgery.  Think again!  WebMD, a leading source in medical news, reports on one study demonstrating the effectiveness of laser eye surgery in older patients.  Citing research from the Massachusetts Eye and Ear Infirmary, the article suggests that “laser surgery corrects vision for patients in [their] 40s, 50s and 60s.”

The study looked at 424 LASIK patients between the ages of 40 and 69.  According to the article, following their surgery, “20/30 vision … was restored in 86% to 100% of eyes corrected for nearsightedness and 80% to 84% of eyes corrected for farsightedness.”  Even those in their 60s?  Yes, WebMD goes on to explain that “Patients in their 60s were just as likely to have good outcomes as those in their 40s and 50s.”

Is LASIK Right for You?

This study brings up an interesting question: Who is a good candidate for LASIK surgery?  While there is no single formula for determining a good LASIK candidate, here are some general guidelines.

A good LASIK candidate:

  • …is in good general health.
  • …has good eye health and is free of eye conditions such as cataracts, chronic corneal infections or keratoconus.
  • …is of a certain age, typically 21 years old.
  • …has a cornea of appropriate shape and thickness.

There are several other factors that need to be discussed before choosing laser vision correction.  The only way to do that is to schedule a consultation with a qualified LASIK surgeon.

Think You’re Too Old for Laser Vision Correction? Study Suggests that LASIK Works in Older Patients

If you’re in your 40s, 50s or 60s, you may think you’re too old to undergo LASIK surgery.  Think again!  WebMD, a leading source in medical news, reports on one study demonstrating the effectiveness of laser eye surgery in older patients.  Citing research from the Massachusetts Eye and Ear Infirmary, the article suggests that “laser surgery corrects vision for patients in [their] 40s, 50s and 60s.”

The study looked at 424 LASIK patients between the ages of 40 and 69.  According to the article, following their surgery, “20/30 vision … was restored in 86% to 100% of eyes corrected for nearsightedness and 80% to 84% of eyes corrected for farsightedness.”  Even those in their 60s?  Yes, WebMD goes on to explain that “Patients in their 60s were just as likely to have good outcomes as those in their 40s and 50s.”

Is LASIK Right for You?

This study brings up an interesting question: Who is a good candidate for LASIK surgery?  While there is no single formula for determining a good LASIK candidate, here are some general guidelines.

A good LASIK candidate:

  • …is in good general health.
  • …has good eye health and is free of eye conditions such as cataracts, chronic corneal infections or keratoconus.
  • …is of a certain age, typically 21 years old.
  • …has a cornea of appropriate shape and thickness.

There are several other factors that need to be discussed before choosing laser vision correction.  The only way to do that is to schedule a consultation with a qualified LASIK surgeon.

Clinical trials aid PSC diagnosis and treatment

Jayant A. Talwalkar, M.D., a hepatologist with the Department of Gastroenterology and Hepatology and the Mayo Clinic Transplant Center in Rochester, Minn., discusses diagnosis, complications, liver transplantation, and Mayo’s role in the development of new diagnostic tests and effective therapies for patients with primary sclerosing cholangitis (PSC).

Dr. Talwalkar notes that PSC is the last major liver disease without effective treatment. There is currently no effective medical therapy to halt disease progression and so avoid complications such as liver failure or bile duct cancer for patients with PSC. PCS is considered an autoimmune condition. It develops silently, most often in patients 40-50 years.

Dr. Talwalkar’s team focuses on clinical trials for the development of diagnostic tests and effective drug therapies for PSC, before transplant is needed. Mayo specialists see several hundred patients with PSC each year.

Eye Consultants of Texas has just launched a new video featuring actual Fort Worth metro area patients telling their stories. The curious metal art object on our wall, explained

Part of building our new Grapevine facility in 2010 included a commissioned piece of art to further add a more interesting, less clinic-like environment for our patients. If you’ve ever been to our office, it’s not hard to miss. It’s located in the main hallway leading to our exam rooms with various eye care instruments, eye charts, and Eye Consultants of Texas elements in steel.

Since we unveiled this piece of art we’ve had lots of questions, including “how was that done,” and “was it hard to do?” The entire piece is actually made up of several individual pieces installed on the wall to create a textured effect. Each was hand drawn onto steel, cut out with a water torch, then finished to give it a copper-like appearance to coordinate with the copper throughout our building.

We simply wanted something different without being too “out there.” We’re glad our patients appreciate that it helps makes our Opthalmology office feel less like a medical facility; especially since it’s made of steel, and not so easy to remove!
