Simplifying LASIK Basics – Part 1: Starting Points

As we’ve mentioned here before, LASIK has become a household word to anyone relying on glasses or contact lenses for everyday things. There’s no shortage of info out there, and much of it can be confusing, or written like an owner’s manual. At Eye Consultants of Texas, we cover the bases in educating our Dallas/Fort Worth LASIK patients, and try to do this as simply as possible. With that in mind, this is the first in a series of blogs we’ll be posting to highlight the basics of LASIK, in hopes of putting LASIK medical-speak more into simple-speak.

First Things First. LASIK is a simple, painless procedure that can be performed quicker and safer than ever before, mostly thanks to technological advancements. LASIK is also surgery on your eyes, so you owe it to yourself to understand the benefits, risks, and everything else about it. And to choose a skilled doctor who’s on your side.

How Long Has LASIK Been Around? A question to this that often follows is, “how many LASIK procedures have been performed?” To answer this objectively, we’ll go to a reputable source. According to the American Academy of Ophthalmology (AAO), LASIK was first FDA-approved in 1998. Since then, the AAO estimates that nearly 10 million procedures have been performed in the U.S. (an average of 700,000 each year). Worldwide, this number is double by some estimates you might find.

Why Would Someone Get LASIK? People who have LASIK performed are usually those that have to rely on glasses or contact lenses for everyday activities. Sometimes they opt for LASIK so they can see better and look better, or simply because they’re tired of having to wear glasses all the time. Sometimes LASIK may be the best solution if corrective eyewear is no longer doing the trick. Whatever the reason, it’s important that your vision health and overall health checks out, that you’re fully informed, and that you’re over 18 years of age. In other words, you have to be a candidate for LASIK to have it done. If for any reason you’re not, there are other options your doctor can tell you about.

Look for more in our next Simplifying LASIK Basics blog coming soon. Meanwhile, if you’d like to know more about LASIK at Eye Consultants of Texas, please call our office in Grapevine at 877-516-4364, conveniently located to Dallas/Fort Worth.

What is Natural Cycle IVF?

Conventional IVF Today’s conventional IVF is ‘stimulated’ – drugs are used to prompt the woman’s ovaries to produce several eggs, rather than just one. The eggs are mixed with her partner’s or a donor’s sperm to become fertilised. Women take a course of ovarian-suppression hormones to shut down their menstrual cycle to control ovulation. Then [...]

Careers As Advanced Health Care Practitioner In Demand

According to data collected by the American Medical Group Association’s and Cejka Search 2011 Physician Retention Survey, careers in the advanced health care field are growing rapidly.  As such, employment in medical groups for nurse practitioners (“NPs”) and physician assistants (“PAs”) has increased significantly in the past five years.  According to the data, 75% of the responses to the survey indicated more NPs and PAs will be hired in the next five years.

Growth in these advanced health care professions is due largely to economics, according to an article in U.S. News and World Report.  NPs, for example, can provide patients with many primary health care services at a fraction of the cost of an internist.  In addition, FiercePracticeManagement reports that due to physician shortages and the demand to create a medical “team” to care for patients (accountable care organizations, “ACO”), medical groups are employing the NP and PA at increasingly high rates.  This gives NPs and PAs the advantage of choosing where to work, thus creating a high turnover rate for these positions.  In addition, recruiting and retaining these professionals are challenges to medical groups.

Books Feed Your Head

It’s true, books really do, “Feed Your Head”.  In 1991 there was an award winning MTV campaign that promoted reading.  It was a revolutionary, ground breaking, and cutting edge style of mixing literature with a short film.  This was when MTV still contained the word “music” in the logo, Tom Freston was the President and CEO of the network, the influence of Bob Pittman was still around, and it offered up value and creativity to viewers.  Unfortunately it seems those days have passed.

Because of this campaign to promote reading I was introduced to the works of Franz Kafka and more importantly the works of Donald Barthelme.  To me, Donald Barthelme is the master of the short story.  I read almost every single day.  I will admit that there are days when I am on vacation that I might miss the opportunity to read in exchange for the opportunity to explore a new city.

Many years ago, Jim Rohn (author and personal development trainer) said, “If you read one book every month about your industry, in 10 years you’ll have read 120 books. That will put you in the top one percent of your field.”  Mr. Rohn was 100% correct and he was just talking about the lazy way to do it.  I once heard someone say, “Skip a meal, but never skip the opportunity to read.”  If you just invest 20 – 30 minutes a day in yourself, to read, there isn’t any reason that can’t read at least one book a week.  If you were to read one book a week that would mean at the end of year you would have read 52 books.  At this rate you would be in the top one percent in two years instead of 10.

For myself, I find that reading in a 80/20 ratio works best for me.  Eighty percent of the books that I read are for knowledge and learning.  Twenty percent of the books that I read are for humor, pleasure, and escapism.  I would personally get bored if all I did was read for learning.  I also need to be entertained when I read.

Please don’t tell me you don’t have enough time to read.  If you really don’t think that you have the time to read…then I suggest that you at least make the time to read, “The 168 Hour Week” by, Dr. Kevin Hogan.

You can find the time to read.  You just have to do it.  I truly believe that reading is a great exercise that you can and should do for your brain everyday to keep it healthy and active.  Think of your brain as muscle…exercise it and make it grow.  The dividends that it will pay you can be huge.

If you’re looking for a new author to read or you want to start reading a new genere of books, then I suggest you take a look at the works of Thorne Smith.  You can find out more about Thorne Smith at the blog and and website created by, Michael Walker at   http://www.thornesmithblog.com/

Grab a book and start reading today.  Remember, “Books Feed Your Head”.

Generic Versus Brand-Name Drugs

Generic drugs account for almost 80 percent of all prescribed medications in the United States and most states allow generic drugs to be dispensed in place of brand-name drugs.  This is quite a large number of generic drugs being taken by quite a large number of the nation’s population.  The possibility of people who take generic drugs and having reactions to the drugs is also large – unfortunately, this is what is happening across the country.  People, who are taking generic drugs, are having severe complications from taking the drugs.  Reactions to these generic drugs are very serious, with some consumers developing inflammatory bowel disease, gangrene, and even movement disorders.

What is even more distressing is that the manufacturers of these generic drugs are protected by a U.S. Supreme Court ruling.  In a 5-4 decision, the Supreme Court ruled in June 2011 that makers of generic drugs cannot be sued for not including side-effect warnings on their labels, if the warnings are also not on the labels of their brand-name equivalent.  This makes it difficult for consumers who may not know they are receiving a generic form of a drug, such as in a hospital, or may be required by their insurance plan to fill a prescription with a generic drug.  If they develop a medical issue while taking a generic form of a prescription drug, they basically have no legal remedy.

The Supreme Court ruling is based on the Hatch-Waxman Act of 1984.  This law allowed companies to skip the involved process required to approve new drugs, if they could prove that the generic drug was equivalent to its brand-name counterpart.  However, Representative Henry A. Waxman, who co-wrote the Act, has stated that “Congress did not intend for consumers’ rights to be categorically eliminated simply because they purchased a generic rather than a brand-name drug.”  Mr. Waxman is investigating a change in the ruling.

Consumers should protect themselves and do their own research before taking any drug – check with your doctor about generic versus brand-name prescriptions and find out about all the possible risks and side effects.

3 Eye Conditions You Need to Be Thinking About

As we age, it’s not uncommon to face all different types of health concerns and challenges.  And, when it comes to your eye health, there’s no exception.

Did you know that eye disorders are much more common among those of a certain age?  We’re not just talking about needing a stronger eyeglass prescription, but rather diseases that can completely reduce your ability to see if left untreated.  Want to know what to watch out for?

Here is our list of the Top 3 Age-Related Eye Conditions You Need to Be Thinking About:

  1. Macular Degeneration: Age-related macular degeneration (AMD) can present itself in different ways – from a blurry area of vision to a dark blind spot.  The condition occurs when arteries that nourish the retina harden. Now deprived of vital nutrients, these retinal tissues begin to weaken, which ultimately causes vision loss.  As the number-one cause of vision loss in the US, any AMD symptoms need to be addressed as soon as possible.  The best way is to schedule a Macular Degeneration Evaluation.
  2. Cataracts: Cataracts are one of the most common eye concerns for those over the age of 65.  The condition, which affects millions of people each year, causes a clouding of the eye’s natural lens.  Often, the only good cataract treatment is surgery.  During a typical Cataract Surgery, the eye is numbed and a small ultrasonic probe is inserted.  This probe breaks up and removes the affected cloudy lens.  A clear new artificial lens called an IOL is then implanted into the eye.
  3. Glaucoma: Glaucoma is a common eye condition, commonly associated with an increased pressure in the eye.  This increased pressure can damage the optic nerve and ultimately cause a gradual but serious loss of vision.  Untreated, the disease often results in complete blindness.  While not curable, modern medicine offers several advanced Glaucoma Treatments.  These include medications, eye drops and sometimes, surgery.

Mayo Clinic offers newly approved treatment for GERD

C. Daniel Smith, M.D. , chair of the Department of Surgery at Mayo Clinic in Florida, discusses minimally invasive surgical options for patients with gastroesophageal reflux disease (GERD).  A novel device, a ring of tiny magnetic titanium beads that acts to keep stomach acid from leaking into the esophagus, is now offered at Mayo Clinic in Florida. Read more about the LINX Reflux Management System in the April 11, 2012, edition of MedCity News.

Mayo Clinic in Florida is one of the first health care institutions in the U.S. to offer a newly approved device to treat GERD. Mayo Clinic in Florida helped test the device in patients. The U.S. Food and Drug Administration (FDA) approved the device and treatment procedure on March 22, 2012, for patients with GERD who continue to have chronic reflux symptoms despite taking medication. Mayo Clinic in Florida was one of 14 centers nationally that participated in a clinical trial that led to the FDA’s approval of the device.

About GERD
In patients with gastroesophageal reflux disease, liquid or food in the stomach flow back up into the esophagus due to the inability of a ring of muscle between the lower esophagus and the top of the stomach to close properly. If drugs aimed at neutralizing the acid in the stomach fails to prevent GERD, an operation designed to correct the mechanical defect is considered. Nearly 2 million patients of those patients could benefit from this treatment, which is much less complex than current surgical options, says Dr. Smith.

The results of the clinical study that led to approval of the device have not yet been published. “The data presented to the FDA, however, revealed striking results when compared to other GERD treatments that have been investigated over the past 20 years,” says Dr. Smith. “The system offers effective control of GERD with limited side effects and thus far an excellent safety record.”

The implanted device is a ring of tiny magnetic titanium beads that is wrapped around the junction between the stomach and esophagus, serving as a mechanical augmentation of the lower esophageal sphincter (the ring of muscle). The magnetic attraction between the beads is strong enough to keep the sphincter closed to refluxing acid, but weak enough so that food can pass through it into the stomach. The device can be implanted using minimally invasive surgery methods.

“I expect this device to be a game changer for the treatment of GERD in select patients who have failed management with drugs,” says Dr. Smith.

Kenneth R. DeVault, M.D. , chair of the Department of Internal Medicine at Mayo Clinic in Florida, also participated in the studies. “I have many patients who are searching for something more than medication for their reflux, but have been hesitant to undergo a traditional reflux surgery,” he says. “I think this procedure may well be a very attractive option for that group.”

Drs. Smith and DeVault were consultants to the company that developed the device and participated in the research study. Mayo Clinic licensed related technology to the company in exchange for equity.

Vast Majority of LASIK Patients Satisfied, Reports ESEC

LASIK, which stands for laser-assisted in situ keratomileusis, has become one of the most popular vision correction surgeries in the world, with thousands of procedures performed every year.  The numbers, however, don’t paint the whole picture.  A surgery’s true measure of success is how many patients are satisfied with their results!  The Eye Surgery Education Council (ESEC), a leading provider of eye surgery information, is reporting on just that.

“LASIK vision correction has the highest patient satisfaction rate of any elective surgery,” explains the ESEC, “95.4%, according to a 10-year survey of scientific studies from around the world.”  Clearly, this degree of patient satisfaction speaks volumes.  Furthermore, the ESEC reports that “In a recent study, about 97% of LASIK surgery patients said they would recommend it to a friend or relative.”  While LASIK is not for everyone and there are risks and benefits to consider, these figures certainly speak for themselves.

LASIK Results

So, what can LASIK do for you?  First, let’s take a look at the procedure itself.  LASIK eye surgery improves vision safely and precisely by reshaping the cornea to correct nearsightedness (myopia), farsightedness (hyperopia) and astigmatism.  The procedure is tailored to your specific medical needs.

Of course, the goal of every LASIK procedure is 20/20 vision.  Looking at one of the most common eye issues, nearsightedness, the ESEC reports that “Studies of mildly or moderately nearsighted patients show that one year after having LASIK surgery: 98% have obtained 20/20 vision.”  So, do you think LASIK may be right for you?  The only way to find out is to schedule a consultation!

Baltimore Eye Surgeon Now Performing Cosmetic Eyelid Surgery

lasik, eye surgery, baltimore md, eyelid surgery, eye surgeon

Baltimore, MD – According to the Administration on Aging (AOA), persons 65 and older represented nearly 13% of the U.S. population in 2009, and are expected to represent 19% by 2030. The Bureau of Labor Statistics finds that the number of older Americans in the workforce is growing as well. As the population and workforce ages, Baltimore eye surgeon Jay C. Grochmal, MD says his patients that are late stage baby-boomers are under extreme pressure to maintain a high level of performance in the workplace, and portraying a vivacious and youthful appearance is essential to continued success.

While helping patients improve their vision with procedures such as LASIK and cataract surgery is important to success in the workplace, Dr. Grochmal says the desire to help his patients enhance their aesthetic appearance has become an important part of his practice as well. He says this growing demand for increased confidence and a natural-looking enhancement is what led him to the decision to add cosmetic eyelid surgery to his list of procedures: “Our society places such a high value on appearance and equates a youthful look with the likelihood of business and social success. Baby boomers are still very active and productive and a refreshed look can eliminate unwarranted bias in career choices.”

As the number of cosmetic procedures performed in the U.S. continues to increase, Dr. Grochmal says the desire to look younger longer is obvious, adding that a tough economy and high unemployment rate further contribute to the stress his older patients must endure. Because he constantly strives to improve patient care while providing the latest and safest procedures, Dr. Grochmal says providing cosmetic treatments for his patients is a result of his original commitment to provide the most effective treatments available to meet any and all needs of his patients: “As the cliche goes now, 50 is the new 40 and 60 is the new 50, and so on. My patients want to look as young as they feel and be able to see well too. Ultimately, we believe providing cosmetic eyelid surgery will improve their chances of success in the workplace.”

About Jay C. Grochmal, MD

After earning his medical degree from the University of Maryland, Dr. Jay C. Grochmal completed his residency while achieving the rank of Chief Resident at the Greater Baltimore Medical Center’s Department of Ophthalmology. In addition to being on staff at some of the leading hospitals in the Baltimore area, Dr. Grochmal has traveled on a number of missions dedicated to improving eye care around the world.

Grochmal Eye Center is located in the Catonsville Professional Center at 405 Frederick Rd., Ste. 102, Baltimore, MD 21228, and can be reached at (410) 697-4090. Dr. Grochmal and his team can also be reached at grochmaleye.com or their Baltimore Eye Care Facebook page.

Medicare And Medicaid Overpayment Suit Upheld

Whistleblowers, Lucas Matheny (“Matheny”) and Deborah Loveland (“Loveland”), brought an action against Medco Health Solutions (“Medco”) claiming that Medco and its subsidiaries failed to report and refund to the federal government $69 million in Medicare and Medicaid overpayments.  The plaintiffs, former employees of Medco, used the authority of the False Claims Act to bring their lawsuit.  The False Claims Act allows private individuals to sue an entity on their own and the government’s behalf.  In doing so, the individuals receive compensation for being a “whistleblower”, which is usually between 15% and 25% of the funds recovered, including reasonable expenses, attorney fees and costs.

Matheny and Loveland knew of the overpayments to Medco, but were told that because of insufficient manpower to process the necessary forms, the overpayments would not be refunded to the government.  Matheny and Loveland claimed the overpayments were transferred to fictitious patient accounts and perfect records with a zero percent error rate were created using a specific computer program.   In addition, Medco filed a false certificate of compliance with the government, knowingly concealing millions of dollars in overpayments.

When the lawsuit was first heard by the court, it was dismissed, due to the court noting that the plaintiffs’ complaint had been amended three times and failed to state a claim. However, on appeal, the court concluded that Matheny and Loveland had sufficiently alleged the existence of a false record.  Additionally, the court held that the defendants (Medco) knew the records were false and were to be used to avoid a financial obligation to the government.

The Matheny v. Medco Health Solutions matter is a prime example for healthcare providers to be careful with potential overpayments and how to handle those overpayments should they occur.

Rabies Vaccine Shortage in the UK

There is at present an acute shortage of rabies vaccine throughout the UK. There will be no further supplies for some time.

Globe Travel Health Centre has sufficient stocks for the time being. However, to conserve stocks:

  • Rabies vaccine will be available on Tuesday, Thursday and Saturday clinics only.
  • The vaccine will be administered by the intradermal route, using a small dose of vaccine.

This is just as safe and efficiacious as the more traditional intramuscular route, but uses less vaccine.

The World Health Organisation has stated:

‘Intradermal immunisation is an acceptable alternative to standard intramuscular vaccination. Intradermal vaccination has been shown to
be as safe and immunogenic as intramuscular vaccination, yet requires less vaccine. This alternative should be considered in settings constrained by cost and/or supply issues
’.

Intradermal rabies vaccination costs £30.00 per dose – a course of three doses is required for primary vaccination.

Top 5 Myths of Glaucoma

Most people have heard of glaucoma, but few know the whole story.  In short, glaucoma is an eye condition where the optic nerve becomes damaged, usually as a result of increased pressure in the eye.  This damaging of the optic nerve ultimately causes a gradual but serious loss of vision.  Untreated, the disease often results in complete blindness.

Now that you have a basic understanding of glaucoma, let’s clear up a few of the most common myths surrounding this serious condition.

Here are the top 5 myths of glaucoma:

1. Myth:  Glaucoma is rare.
Fact:
On the contrary, it has been estimated that over 2 million Americans are suffering from glaucoma.  Even more disturbing is the fact that glaucoma remains a leading cause of blindness in the US.

2. Myth:  Only patients with a family history can develop glaucoma.
Fact:
Although certain factors like age and heredity can increase your chances of developing glaucoma, the condition can affect anyone, anywhere and at any age.

3. Myth:  Glaucoma cannot be treated.
Fact:
While there is currently no cure, there are a host of Glaucoma Treatments aimed at minimizing the risk of permanent vision loss. These treatments include medications, eye drops and surgery.

4. Myth: Only patients with glaucoma symptoms should get checked out.
Fact:
The majority of glaucoma patients do not experience any symptoms before the optic nerve actually becomes damaged.  For this reason, regular glaucoma screenings are vitally important.

5. Myth:  Glaucoma screenings are painful.
Fact:
Glaucoma screenings test different things—visual field, pressure in the eye, etc.  Each of these tests are not only quick, but non-painful.

Medicare Star Quality Ratings Released

The Centers for Medicare & Medicaid Services recently announced that of 563 Medicare Advantage plans rated nationwide, only 11 Medicare health plans with prescription drug benefit earned 5 stars for 2013 — the highest overall rating for quality and service.

Each year through the Medicare Star Quality Ratings system, CMS rates Medicare health plans (both parts C and D) on a scale of 1 to 5 stars, with 5 stars representing the highest quality. The overall scores are based on more than 50 care and service quality measures across five categories, including staying healthy, managing chronic conditions, member satisfaction, customer service and pharmacy services.

In addition to receiving high-quality care and service, 5-star excellence means that Medicare beneficiaries may enroll in a 5-star plan from Dec. 8, 2012, to Nov. 30, 2013, without having to wait until the next annual enrollment period.

Leading the nation with six of the 11 Medicare health plans earning 5 stars was Kaiser Permanente. The Kaiser Permanente 5-star Medicare plans are those operating in California, the Northwest, Hawaii, Colorado, Ohio and the Mid-Atlantic States. Kaiser Permanente’s Georgia region is the top-rated plan in its area with 4.5 stars.

Medicare beneficiaries can learn more about the Medicare Star Quality Ratings by visiting kp.org/medicarestars or medicare.gov, or by calling 1-800-MEDICARE (1-800-633-4227).

Using EHR Data to Measure Improvement: Interviews with Experts

During Health IT Week, an event held at the Center for Total Health—sponsored by the U.S. Department of Health and Human Services’ Office of the National Coordinator for Health Information Technology, the National Quality Forum, and Kaiser Permanente— examined the state of electronic measurement in health care, lessons learned from the Beacon Community Program, and what’s next in meaningful use. While at the event, we were able to speak briefly with a few of the presenters about the topic. This video features interviews with Kevin Larsen, MD, Craig Brammer, and Kerri Petrin, all three of whom are with the ONC.

Cost of LASIK versus glasses or contacts

Since the first LASIK technology was FDA-approved, well over a decade ago, LASIK has gone from little known treatment to household name. If you’ve depended on glasses or contacts for most of your life, chances are that you’re familiar with LASIK’s benefits. Our LASIK patients across Dallas/Fort Worth certainly come to us more savvy about this procedure than ever before.

There are the obvious benefits. LASIK is a simple procedure that can add years of improved vision. However, although LASIK is elective (and not covered by insurance), there can be cost benefits as well. On average, LASIK can actually cost less than wearing glasses or contacts over time. Naturally, this can depend on things like how often you have to buy contacts or change your prescription; related doctor visits, and the age you have LASIK performed.

Costs can vary between patients, depending on what is actually required. This is where a thorough eye exam becomes so important. It ensures you’re a LASIK candidate, and helps to pinpoint what needs to be done. More importantly, it takes your vision and overall health into consideration, and allows our doctors to give you the full story.

To get a basic idea of costs versus glasses or contacts, click on the iLASIK Calculator on our LASIK page. To learn more or to set up an appointment, please call our Metro Dallas offices in Grapevine at 877-516-4364.

Changes are Coming to Your Health Savings Accounts

It’s the season of open enrollment for health insurance plans, and if you have an HSA (Health Spending Account – sometimes called a Health Reimbursement or Flex Spending Account), it’s time to max it out! Tax laws have changed, and this coming year, 2012, is the last year that you can place the maximum $5,000 in your HSA. Starting in 2013, the maximum allowed in any Health Savings Account will be $2,500, which means that the entire cost of laser eye surgeries such as LASIK cannot be covered by your HSA pre-tax dollars after 2012.

If you have questions about the coming changes, or if you would like to set up a consultation for LASIK or other laser vision correction procedures, please contact Grochmal Eye Center at (410) 697-4090.

Spine-health Named Top 50 Most Visited Website

Spine-health.com is proud to announce that it was recently ranked one of the top 50 most visited health websites, according to ComScore, a global leader in measuring the digital world.

Spine-health.com joins the large general health websites that make up the bulk of the top 50, and was one of the few “vertical” websites – sites that focus in on a particular aspect of health. Spine-health.com focuses exclusively on the spine, and covers conditions that cause back and neck pain.

This blog is a general overview of LASIK surgery for general informational purposes only, and in no way intended as actual medical advice. Always seek a medical or eye care professional for advisement based on your individual health and eye care needs.

Mayo Clinic study finds that cardiac rehab reduces mortality by 45 percent

Randal J. Thomas, M.D., director of Mayo Clinic’s Cardiovascular Health Clinic, and colleagues found that patients who participate in cardiac rehabilitation after having heart interventions such as angioplasty, stents and clot-busting drugs have a 45 percent lower mortality rate.

The research team studied data from more than 2,300 patients between 1994 and 2008, and recently published the results in Circulation.

Only about 40 percent of patients in the study participated in cardiac rehabilitation. These findings are particularly important for interventional cardiologists, Dr. Thomas says, because encouraging patients to pursue cardiac rehab after their procedure can potentially save more lives than previously thought.
